ARIZONA PUBLIC SAFETY PERSONNEL RETIREMENT SYSTEM

    LOCAL PENSION BOARD MEETING – FIRE/POLICE

JUNE 19, 2008

    A meeting of the Public Safety Local Fire and Police Pension Boards of Mesa, Arizona, was held in Conference Room 870, 20 East Main, 8th Floor, on June 19, 2008 at 11:00 a.m.

    Chairman Whalen excused Boardmember Brady from the meeting.

    1. Approval of the minutes of the Public Safety Local Fire and Police Pension Boards meeting of

    May 15, 2008.

    It was moved by Boardmember Pace, seconded by Boardmember Cluff, that the minutes of the Public Safety Local Fire and Police Pension Boards meeting of May 15, 2008 be approved.

    Chairman Whalen declared the motion carried unanimously by those present.

    FIRE PENSION BOARD

2. Discuss and consider the DROP application of Deputy Fire Chief Fernando Valenzuela.

      It was moved by Boardmember Perkins, seconded by Boardmember Cluff, that the DROP application of Deputy Fire Chief Fernando Valenzuela be approved.

    Chairman Whalen declared the motion carried unanimously by those present.

    3. Discuss and consider the Accidental Disability Retirement application of Fire Captain Mike Dubois.

      Chairman Whalen welcomed Fire Captain Mike Dubois to the meeting. Chairman Whalen advised Captain Dubois that the members of the Local Board have reviewed his application and medical records. Chairman Whalen outlined the process that would be followed by the Board and encouraged the members to pose any questions they may have regarding Captain Dubois’ pending Accidental Disability application.

      Chairman Whalen stated that in view of the absence of questions from the Board, he would entertain a motion relative to this case.

      It was moved by Boardmember Perkins, seconded by Boardmember Cluff, that pending disability applicant Fire Captain Mike Dubois be referred to a Local Board physician for an independent medical evaluation.

    Chairman Whalen declared the motion carried unanimously by those present.

    Without objection, the meeting of the Local Fire Pension Board adjourned at 11:13 a.m.

    POLICE PENSION BOARD

    4. Certification of full-time Law Enforcement Officers pursuant to A.R.S. §§38-842.14(A) and 38-

      847(D)1.

          Subsequent to a review of all medical reports, Local Boardmembers requested that the record reflect that the following applicants have pre-existing medical conditions: Ashley Beyer, Glen Huber, Adam Kalik, Michael Leo, Justin Organ, Alan Pak, Darrel Palmer, and Andres Sanchez.

          It was moved by Boardmember Pace, seconded by Boardmember Smith, that the following eight (8) law enforcement officer applicants, Ashley Beyer, Glen Huber, Adam Kalik, Michael Leo, Justin Organ, Alan Pak, Darrel Palmer, and Andres Sanchez be certified pursuant to A.R.S. §§38-842.14(A) and 38-847(D)1.

      Chairman Whalen declared the motion carried unanimously by those present.

5. Discuss and consider the DROP application of Police Officer Ron Martinez.

      It was moved by Boardmember Smith, seconded by Boardmember Shelley, that the DROP application of Police Officer Ron Martinez be approved.

      Chairman Whalen declared the motion carried unanimously by those present.

6. Discuss and consider the DROP application of Police Officer Steven Raz.

      It was moved by Boardmember Smith, seconded by Boardmember Shelley, that the DROP application of Police Officer Steven Raz be approved.

      Chairman Whalen declared the motion carried unanimously by those present.

7. Discuss and consider the election of Police Officer James Mills to terminate his participation in the DROP program and apply for Normal Retirement.

        It was moved by Boardmember Smith, seconded by Boardmember Pace, that the election of Police Officer James Mills to terminate his participation in the DROP Program and apply for Normal Retirement be approved.

      Chairman Whalen declared the motion carried unanimously by those present.

      (The Board recessed at 11:17 a.m. in order to convene an Executive Session and reconvened the Public Safety Local Police Pension Board meeting at 11:19 a.m.)

    8. Discuss and consider the Accidental Disability Retirement application of Police Officer Royed Hollick.

      Chairman Whalen welcomed Police Officer Royed Hollick to the meeting. Chairman Whalen advised Officer Hollick that the members of the Local Board have reviewed his application and medical records. Chairman Whalen outlined the process that would be followed by the Board and encouraged the members to pose any questions they may have regarding Officer Hollick’s pending Accidental Disability application.

      Chairman Whalen indicated that Officer Hollick’s application is lacking a medical certification relative to his disabling injury as it relates to his ability to perform his job.

      Chairman Whalen stated that in view of the absence of questions from the Board, he would entertain a motion relative to this case.

      It was moved by Boardmember Pace, seconded by Boardmember Smith, that the pending Accidental Disability retirement application of Police Officer Royed Hollick be continued until additional medical documentation is received and can be reviewed by the Local Board.

      Chairman Whalen declared the motion carried unanimously by those present.

          Chairman Whalen expressed his appreciation to Boardmember Joseph Shelley for his service to the Board.

    9. Adjournment.

      Without objection, the meeting of the Local Police Pension Board adjourned at 11:24 a.m.

I further certify that the meeting was duly called and held and that a quorum was present.
